A definitive and compelling book on one of today's most prevalent illnesses. In 2020, an estimated 5.8 million Americans had Alzheimer’s, and more than half a million died because of the disease and its devastating complications. 16 million caregivers are responsible for paying as much as half of the $226 billion annual costs of their care. As more people live beyond their seventies and eighties, the number of patients will rise to an estimated 13.8 million by 2050. Part case studies, part meditation on the past, present and future of the disease, The Problem of Alzheimer's traces Alzheimer’s from its beginnings to its recognition as a crisis. While it is an unambiguous account of decades of missed opportunities and our health care systems’ failures to take action, it tells the story of the biomedical breakthroughs that may allow Alzheimer’s to finally be prevented and treated by medicine and also presents an argument for how we can live with dementia: the ways patients can reclaim their autonomy and redefine their sense of self, how families can support their loved ones, and the innovative reforms we can make as a society that would give caregivers and patients better quality of life. Rich in science, history, and characters, The Problem of Alzheimer's takes us inside laboratories, patients' homes, caregivers’ support groups, progressive care communities, and Jason Karlawish's own practice at the Penn Memory Center.

Who could deny the devastating effects of dementia on the lives of both caregivers and those afflicted? By tackling a book on the personal effects and political background of Alzheimer's & dementia, Karlawish is faced with an opportunity for praise and respect by highlighting details of a disease that have mostly gone unmentioned. Yet, I was left unsatisfied. No doubt, the story of Alzheimer's research is a story worth telling, but beyond the matter of fact knowledge presented in the book lies a more frustrating example of white male privilege in science and the deep troubles our country continues to face with medical establishment personalities. The book reads like a father-to-son recollection of the "glory days" in a research career. It is replete with dubious male-centered subtitles: "Young Men and Science", "Two Men, One Molecule, and Countless Fishing Trips", "A Young Man in a Hurry", "A Self-Help Group for the Self-Made Man". I winced at his chapter introduction: "No One Says No to Len Kurkland". Unfortunately this is a common attitude in medicine. "This person is an established profession and you must not doubt his authority!" Emphasis his. I wonder, would people ever say no to Barbara Kurkland? Even if they wouldn't, should an established medical professional be encouraging this type of unquestioning loyalty to another professional? Is it not worth at least pointing out these problematic tendencies in the author? The hardcover is colored blue by the way (B is 4 boys!), why not purple, for Alzheimer's disease which disproportionately affects women who are often stuck at home preparing for their male spouses to arrive from work thus increasing their risk for Alzheimer's due to social isolation? Beyond the numerous male-centered analogies scattered throughout the book, the quotes at the beginning of each chapter paint the picture, yet again, of an uncultured white male. Quotes range from The Constitution, to various quotes from scientists (mostly male), an occasional poem (respectable), the Oxford English Dictionary (seriously?), not one but four Bible quotes (nothing from the Quran, Tanach, Sutras, or Vedas), and Dante's Inferno (a white Christian male's idea of hell). One thing is for certain, it felt like hell looking past some of these issues, but ultimately, we should, because despite the glaring flaws, Karlawish does address an issue in a way that hasn't been done before. In this sense, it is worth the read, especially if you are interested in Alzheimer's research, but let's all take some time to consider more deeply the perspectives that are running the hospitals in the United States every day.
